English: Dear Followers, Hormone replacement therapy (HRT) as a remedy for the symptoms of the menopausal transition has been (and partly still is) one of the most discussed and controversial topics in gynecological practice. In fact, the fluctuating opinions over the years on the potential benefits and potential harms of HRT have led to substantial changes in its use, moving from a systematic prescription, based on the progressive increase in the part of life spent in climacteric hypoestrogenism, to a general distrust and under-prescription caused by the results of studies conducted at the end of the last century, with characteristics of the participants that only partially respected our clinical habits. According to the 2022 Hormone Therapy Position Statement of the North American Menopause Society, the current indications for systemic HRT in menopause concern the treatment of vasomotor symptoms, genitourinary syndrome and the prevention of bone loss. The menopause period represents a complex transition and the benefits of HRT could prove important, provided that precise selection criteria of the candidates are respected. More generally, considering that the impact on health-related quality of life, sexual function, bone metabolism and cancer risk concerns a progressively increasing portion of the population, HRT today has the characteristics of social medicine. On Saturday 2 December we had the privilege of hosting a truly exceptional panel of experts on this topic. Professor Rossella Nappi, full professor of gynecology at the University of Pavia and president of the International Menopause Society, described the peri-menopausal cardio-metabolic transition. Professor Angelo Cagnacci, full professor of gynecology at the University of Genoa, illustrated the current guidelines on the indications for the use of HRT. Finally, Professor Tommaso Simoncini, full professor of gynecology at the University of Pisa, discussed genitourinary syndrome and osteoporosis in menopause.
