On 24 August 2001, Air Transat flight TSC236, an Airbus A330-243, was flying from Toronto to Lisbon when a fuel leak from the right engine began 3 hours 46 minutes into the flight, going undetected. The crew, noticing the fuel shortage, diverted the flight to Lajes Airport, Azores. 150 miles from the destination, the right engine failed, followed by the left at 65 miles. With both engines out, they made an emergency landing at 06:45 UTC. There were 18 injuries and structural damage. The leak was due to a crack in the fuel line from incomplete maintenance.
