I know that in the internship they tell you that you should wait for the circulating nurse to open the table while you brush your teeth and get dressed. However, in the rush of everyday life, ask the circulating nurse if he or she would like your help opening the table. Because, in some hospitals, the circulating nurse can only dedicate himself or herself to opening the materials after helping the anesthetist to get the access and anesthetize the patient. However, during this time, if you go to brush your teeth, you will come in and nothing will be open. This delays the entire process for you to set up the table. And remember, after anesthetizing the patient, the doctor is already coming in to start the surgery. Everything is very fast and rushed after the patient is anesthetized. Therefore, I recommend opening everything first, so that while the circulating nurse helps the anesthetist, you can already set up your table. It all starts by opening the lap, the package that contains the surgical drapes. It can be made of fabric (to be sterilized) or disposable. In the video, I show opening the fabric (made of cotton). Then, I open the surgical gowns that already come with the compress to dry your hands after surgical antisepsis and enter the room. First, be very careful when opening the instruments, so that they do not touch you during the opening process. The only part that can touch is the inside, as it is also sterile. Then, I start by opening an instrument that will be used: the Doyen valve. Usually, the CME packs it in two packages, for safety. You can choose to open both or just one and leave the other closed on the table. I have already opened both here, protecting my arm and placing it on the table. Now for the box, we usually remove the label that is stuck on it, which contains the number of clamps, the box number and this is the control that the circulating nurse has to check if at the end of the surgery, the number of clamps matches the initial number. I always open it against me, leaving it completely open, holding the box, protecting it by covering my hand to hold its weight and resting it on the sterile table. In the kidney basin kit, domes and handle, there is a basic scheme for disassembling it without getting contaminated. By leaving these items open and separate, the circulating nurse can already put the chlorhexidine (for patient asepsis), the methylene blue in the domes and use the kidney basin to make solutions or keep only the serum. You protect your hand with the sterile tissue and use the inside to pick up the materials and turn them over in a safe position on the table. Thus, leaving it free for use. Opening a pharmacy material, the waterproof field, which is placed under the table where the materials will be assembled. You open the packaging and hold it from the outside, fold it and place it on the table where the materials are being opened. An example for opening compresses. In addition to the sterile packaging, it comes in another blue packaging. In this case, if you want to leave a package of compresses open, outside the blue packaging, or if you happen to touch it and “contaminate” that part, you will no longer be able to go to the sterile table. So, open the package, protecting your hand and pour the loose compresses on top of the field.
