Trevelyan says that he was a smart student but did not have enough money, but a man named Blessington offered to help him, as he helped him open a clinic and paid all his expenses, and in return he stipulated that Trevelyan give him three quarters of what he earned per day and live with him. Everything was going normally, and Blessington showed that there was always a doctor by his side, but suddenly, something happens to Mr. Blessington, as he begins to act strangely and disturbed. Shortly after that, two men come to Dr. Trevelyan, who turned out to be Russians, an old man suffering from cataplexy with his eldest son, the doctor begins to examine the patient, but the two men suddenly leave in a suspicious manner. The same two men return the next day, justifying their behavior the day before by saying that they thought the examination was over, but the eldest son notices that his father is not feeling well, so he decides to bring him to the doctor again. While they are in the clinic, Mr. Blessington is out doing his usual rounds, and when he returns he says that his room has been broken into, but nothing has been stolen. Holmes and Watson, accompanied by Trevelyan, go to see Mr. Blessington, who appears terrified, and even threatens Holmes and Watson with a pistol, but Trevelyan convinces him that they are there to help him, not to harm him. Blessington refuses to tell Holmes the truth, and merely admits that he has a quantity of money in his room, as he does not trust banks. The next morning, Blessington is found hanged. Initial police investigations indicate that it was a suicide, but Holmes seems convinced that Blessington was murdered, and his sights turn to the two strange Russians.
