Spinal stenosis therefore results in compression of the spinal cord. This pressure on the spinal cord results in pain and neurological deficits, e.g. numbness in the legs. The cause can be a congenital narrowing of the spinal canal. However, degenerative changes are far more common, e.g. the formation of bony outgrowths on the vertebrae (so-called 'spondylophytes'). Other causes of the narrowing can be osteoarthritis of the spine or degeneration of the intervertebral discs - the intervertebral discs then lose height. A narrowing of the spinal canal leads to pain in the affected person, which is felt primarily in the lower back area. This is noticeable when walking (so-called 'intermittent spinal claudication'). The symptoms are alleviated when the upper body is bent, for example when cycling. The forward curvature of the spine causes the individual vertebrae to fan out and the pressure that was previously on the spinal cord disappears. In addition to the pain, neurological deficits can also occur. Such symptoms include, for example, numbness in the extremities or tingling. If left untreated, permanent damage can occur: In addition to deficits in motor skills, paralysis is also possible (so-called 'paralysis'). When is an operation necessary? Operations are particularly necessary when partial paralysis occurs due to spinal canal stenosis (so-called 'paresis'). Surgery can also be seen as an alternative to drug and therapeutic treatment. The decision to undergo surgery depends on the level of suffering. There are various different surgical methods, all of which aim to decompress the spinal canal; the aim is to relieve the strain on it in the long term. This can be done, for example, by removing the bony outgrowths on the vertebrae at the relevant point. Another surgical approach is to remove excess parts of the vertebral arch that are constricting the spinal cord. As with all operations, there are risks. One fifth of these operations are associated with complications.
