At the end of their normal life cycle (about 120 days), red blood cells are eliminated from the circulation. Hemolysis is defined as the premature destruction and, therefore, shortening of the life span of red blood cells. If this increased destruction is not adequately compensated by an increase in red blood cell production, hemolytic anemia results. Hemolytic anemias can be classified as intrinsic (congenital) and extrinsic (acquired). Congenital hemolytic anemias arise as a consequence of inherited abnormalities of red blood cell structures, such as the membrane, hemoglobin, or red blood cell enzymes, and include hereditary spherocytosis, sickle cell disease, thalassemias, and glucose-6-phosphate dehydrogenase and pyruvate kinase deficiencies. Acquired hemolytic anemias result from forces or agents that, through immunologic, chemical, or physical mechanisms, damage the red blood cell. These include autoimmune hemolytic anemias, microangiopathic anemia, severe burns, and some oxidizing agents.
