The symptoms are initially mild and nonspecific. It often begins in adolescence with only menstrual pain, which becomes more constant and strong or symptoms arise that do not suggest menstruation or are no longer related to it. Many women discover it when they are unable to conceive and thought they had intestinal and urinary pathologies or pelvic infections. Endometriosis is often associated with or manifests itself with anxiety and depression, tiredness and difficulty carrying out tasks at school, at home or at work and can cause discomfort, even serious, in the life of a couple. Gynecological examination, even if accurate, and transvaginal ultrasound, may not be able to find endometriosis. Laparoscopy with histological confirmation of endometriosis remains the cornerstone of diagnosis and would be the therapy that can remove it, but it is not prudent, due to the surgical risks and potential damage to fertility, to intervene in all cases. Magnetic resonance imaging is an excellent investigation that can be done, if ultrasound is not decisive, to limit the use of laparoscopy, to avoid its risks and the reduction of the oocyte reserve. In endometriotic patients who are trying to conceive, it is considered preferable to try for pregnancy immediately, with medical assistance if necessary, and to reserve surgery for those with severe pain or deep endometriosis or large endometriotic cysts, which do not respond to hormone therapy. Medical treatment reduces pain, improves quality of life, reduces the risk of infertility, of having to undergo surgery and of having ovarian tumors, if personalized, controlled and continued for as long as possible.
