In Italy, chronic obstructive pulmonary disease, commonly known as COPD, represents a major health challenge, affecting approximately 3.5 million people and causing 55% of deaths related to respiratory diseases. Faced with this scenario, renewed hopes emerge thanks to the new recommendations of the Global Initiative for Chronic Obstructive Lung Disease (GOLD), which provide crucial updates for the diagnosis, prevention and treatment of the disease. These guidelines, periodically reviewed by an international scientific committee, are based on the most recent clinical evidence and are adopted by the main scientific societies in the sector. One of the most significant aspects of these updates concerns the therapeutic approach to COPD. The recommended strategy now begins with dual bronchodilation, combining LAMA (Long-Acting Muscarinic Antagonists) and LABA (Long-Acting Beta Agonists). Furthermore, a switch to triple therapy - including the addition of ICS (Inhaled Corticosteroids) - is suggested in patients with high blood eosinophil counts. This evolution in treatment highlights the importance of early diagnosis and timely intervention. COPD is an inflammatory disease of the bronchi, often related to cigarette smoking, but also to environmental and occupational factors. Its progression can be slowed, but not reversed, making prevention, especially the fight against smoking, crucial. The new GOLD recommendations place increasing emphasis on the need for early and aggressive treatment, as well as on simplifying therapies, for example through the use of combination inhalers. Finally, the major challenge remains to improve patient adherence to treatment and promote early diagnosis. Awareness of COPD and its implications must be increased among both patients and healthcare professionals, to ensure optimal management of the disease and reduce its impact on public health. According to the most recent update of the Gold Global Initiative for Chronic Obstructive Lung Disease, the new first-choice therapy for diagnosing COPD is a combination of two bronchodilators (LAMA and LABA). If a high level of eosinophils is present in the blood (more than 300), a third medication, an inhaled steroid (ICS), is added to the therapy. COPD is an inflammatory disease of the bronchial tubes and is primarily caused by smoking, but can also be caused by environmental and workplace exposure, and pollution. Early diagnosis is crucial as the disease tends to progress over time. The main challenges in treating COPD include early detection of the symptoms and initiation of treatment. Three classes of pharmacological treatment are used for COPD: bronchodilators (beta-2 agonists and anticholinergics), and inhaled steroids. The goal is to implement a stepwise therapy, starting with bronchodilators, and adding steroids to reduce exacerbations and improve symptoms and lung function.
