Triple-negative (TN) breast cancer represents 15% of breast carcinomas and is defined by the absence of the three main biomarkers of breast cancer - that is, lack of expression of estrogen receptors, progesterone receptors, and lack of amplification or overexpression of HER2. This negative definition, together with clinical and biological heterogeneity, has led experts to consider TN breast cancer as a title of convenience, rather than a defined biological entity. From a clinical perspective, TN breast cancer represents a highly relevant subgroup since patients affected by the disease do not benefit from endocrine or anti-HER2 therapy and chemotherapy is the only established therapeutic option. Despite their unfavorable prognosis when considered as a single group, many TN breast cancers are highly sensitive to chemotherapy, and have an increased neoadjuvant response rate compared to other breast cancer subtypes. This phenomenon is commonly referred to as a triple-negative paradox, because although highly proliferative tumors have a poor prognosis, there is a high chance of response to chemotherapy.
