Welcome back to the last meeting of the Mangiagalli Journal Club of 2022. Uterine adenomyosis is a widespread but probably often under- or misdiagnosed pathology. Scientific evidence suggests a reduced probability of conception and an increased risk of miscarriage in the presence of adenomyosis, both in the case of natural conception and after in-vitro fertilization. Furthermore, according to the results of some systematic reviews of the literature, adenomyosis could also have an impact on the subsequent course of pregnancy. The first, fundamental step in trying to better understand this complex relationship is to make the correct diagnosis. No one better than Professor Caterina Exacoustos, of the University of Rome-Tor Vergata and an international authority in the field of gynecological ultrasound, could introduce us to the imaging and classification of adenomyosis. Dr. Silvia Vannuccini, now a recognized expert in endometriosis and adenomyosis at the Careggi Hospital in Florence, will talk to us about the pathogenesis of the pathology and the potential inflammatory mechanisms that impact fertility. Finally, Laura Buggio, a young and established researcher in gynecology at the Policlinico di Milano, will delve into the obstetric implications of adenomyosis, defining the consequent counseling methods to be adopted in pregnant women affected by the pathology.
